Sous ce nom qu’on ne comprend pas forcément au premier abord se cache un processus qui permet à une personne atteinte de troubles psychiques d’exprimer ce qu’elle veut et ce qu’elle ne veut plus en cas de rechute. \nCe document est rédigé conjointement par la personne concernée et par un professionnel. Le PCC définit les choix du patient qui sont à observer pour sa prise en charge lors d’une décompensation\, en prenant en compte son expérience et son vécu. \nIl vise aussi à identifier les signes avant-coureurs d’une crise et les facteurs qui la déclenchent\, ainsi que les ressources et les stratégies à mettre en œuvre pour y remédier. \nPar ailleurs\, dans certaines situations\, le PCC permet aussi de prévenir et d’éviter une hospitalisation\, et plus particulièrement une hospitalisation sous contrainte. \nQue vous soyez patients\, professionnels ou proches\, venez à ce rendez-vous derrière vos écrans\, les intervenants répondront à vos questions. \n[/fusion_text][/fusion_builder_column][/fusion_builder_row][/fusion_builder_container]
